Form Details

This form is intended for faculty and staff to request access to 25Live on behalf of students who need to reserve space for academic or student-related activities. Requestors are required to provide students with the University Events Scheduling and Communication training materials to ensure they understand and follow the relevant policies and procedures. University Scheduling and Communications reserves the right to change student access to view-only status if they do not comply with these policies.

Department Request Form for Student Access to 25Live

The name of the faculty/staff member requesting a student be given more permissions within 25Live.
Please fill out a new form for each student type. If all students fall under the same student type, you may enter all the names into the box below.
Please enter the student name(s) and e-mails, separated by a comma. Please enter each student on new line. Example: John Doe, jdoe@appstate.edu
Explain the purpose for why this student needs elevated permissions within 25Live. Please specify if this student will be requesting space on behalf of a department/organization.
How long does this student need elevated permissions in 25Live?
What spaces should the student be able to request/approve? Please be specific by using building names and room numbers.
